2. The receptionist will appraise the patient on the doctor’s availability and their timings. The patient will be registered to under doctor available in the outpatient unit.
3. Patients with referral letter will be registered with the doctor whom they have been referred to.
4. Patient desiring to be registered to any other doctor will have to give a written request.
5. The Receptionist will hand over the “Outpatient Registration Information Form to the patient/attender to fill in the patient’s demographic details which is digitalized. The system automatically generates a unique identification number and a bar-coded Outpatient Card (i.e. smartcard) is issued which is used throughout his/her association with G. Kuppuswamy Naidu Memorial Hospital. It is also used to retrieve the records in the future. The patient is advised to bring this card during review visits.
6. In the absence of the patient, if the attender wants to meet the consultant to get an opinion, he/she needs to follow the same registration process.
7. The Nursing Assistant of the respective department will verify the patient’s documents and will call the patients based on the appointments.
Review registrations :
- Patients can fix their appointments with department concerned at the reception desk
- On review visit patients should bring their outpatient card & all medical reports for review

- Prior appointment can be availed for radiological investigations
